Why Scoliosis Progresses in Children: 5 Factors

Scoliosis is a three-dimensional spinal curvature seen more often in growing children and adolescents. After diagnosis, the question families ask most often is: “Why is the curvature increasing — what makes it progress?” The risk of progression in scoliosis is well defined scientifically. Below, we look at the five main factors highlighted in clinical studies.

The rapid growth period

The most important factor in progression is the child’s rate of growth; as the spine grows, structural imbalances can become more pronounced. The early growth years between ages 9 and 12, and the rapid height gain between 12 and 16, are the stages of highest risk. During Risser stages 0–2, which indicate skeletal maturity, the curvature can progress more quickly. This is why regular follow-up matters in growing children.

The degree and direction of the curvature

The higher the Cobb angle, the greater the risk of progression. As a general framework, 10–20 degrees carries low-to-moderate risk, 20–30 degrees moderate-to-high, and above 30 degrees high risk; above 40 degrees, surgical evaluation may come onto the agenda. In addition, thoracic (upper back) curves, double curves and curves with marked rotation tend to progress faster than single curves. This is why individual assessment is critical.

Brace use and adherence

A brace is an effective tool for limiting progression during the growth period; however, its effect depends on correct use. Wearing it for less than the recommended time reduces its effect. Scientific studies show that in children who wear the brace long enough each day (generally around 18 hours), the risk of progression is significantly reduced. A brace is more supportive when it is worn for sufficient time, fits the skin well, is renewed as the child grows and is carried out together with Schroth exercises. Muscle imbalances and posture habits

Scoliosis is not only about bone structure; it is a three-dimensional condition that also involves the balance of muscle and fascia. Asymmetric activity of the muscles around the spine, sitting for long periods, always carrying a bag on the same side, leaning forward at a screen and weak trunk muscles can all make the curvature look more pronounced and contribute to its progression over time. This is why short, daily Schroth-based work and correct breathing exercises support muscle balance and help manage progression.

Gaps in follow-up

Scoliosis care requires continuity; regular monitoring directly affects how the process unfolds. When follow-up lapses, the curvature can progress unnoticed, the brace can start to become too small, exercise adherence can drop and a growth spurt can be missed. This is why every child with a scoliosis diagnosis should be followed with regular clinical assessment, X-rays when advised by a physician, regular Schroth work and brace follow-up.
