Does Swimming Help Scoliosis? The Evidence

Scoliosis is a three-dimensional spinal curvature in which the spine bends sideways, and it is usually noticed during adolescence. The curve sometimes appears in a single region as a “C” shape, and sometimes on both sides as an “S” shape. One of the most common questions is: “Does swimming help scoliosis?” The popular belief is that swimming will balance the spine; however, scientific data and clinical experience paint a more nuanced picture.

Scoliosis and sport

Every sport works the body in a different way. Basketball, football, volleyball, Pilates or swimming can build muscle strength and endurance and contribute positively to general health. But none of these sports is a scoliosis-specific approach.

Scoliosis is a structural curvature of the spine. The course of the curvature does not change simply through strengthening or stretching the muscles. There is no scientific evidence that swimming, or any sport, directly addresses scoliosis. Sport is certainly valuable — it supports cardiovascular health, muscle strength and quality of life. But general sport and a scoliosis-specific approach need to be kept apart.

Why is swimming seen as a solution?

This belief largely stems from the idea that swimming works the spine symmetrically. It is true that movements in water place less load on the joints and work the muscles in a balanced way. However, these effects do not, on their own, change the course of a scoliosis-related curvature.

For example, in a person with scoliosis, there may be an imbalance between the muscle structure on one side of the trunk and the other. When these groups work in a similar way while swimming, the existing imbalance does not even itself out; the curvature persists in the water too, following the person’s own posture. In other words, swimming provides benefit as general exercise rather than through any scoliosis-specific effect.

The scoliosis-specific approach: Schroth therapy

One of the scoliosis-specific approaches used in scoliosis is Schroth therapy. Originating in Germany, this method addresses the spinal curvature in three dimensions; specific exercises are planned according to the direction and degree of the person’s curve and their postural characteristics. The aims of Schroth work are:

  • Working on curvature-specific posture correction (auto-correction)
  • Balancing the trunk with asymmetric breathing techniques
  • Supporting the muscles that will hold the corrected posture
  • Building accurate posture awareness

These techniques are applied differently for each person. For example, the exercises for someone with a “C” shaped curve to the right differ from those for someone with an “S” type double curve. Should people with scoliosis do sport?

None of this means that sport is bad for scoliosis. On the contrary, sport contributes greatly to general health. People with scoliosis can enjoy swimming, basketball, Pilates or volleyball and support their general fitness. These activities, however, do not take the place of a scoliosis-specific approach.

In summary

Swimming can support the quality of life of people with scoliosis, but it does not, on its own, change the course of the curvature. In scoliosis, individually planned, scoliosis-specific exercises come to the fore, and Schroth therapy is one of the key approaches here.
